PACIFIC COUNTY - On April 16 at 1:59 p.m., a 1995 Peterbuilt truck driven by Brian Keith Shaffer, age 36, of Long Beach, was headed southbound on SR101 near milepost 37, about 12 miles north of Naselle, when the right front tire blew out.

According to a report by the Washington State Patrol, this caused the truck to exit the road to the right.

The truck struck several trees, and came to rest on a stump with the driver pinned in the vehicle. Mr. Shaffer was pronounced dead at the scene.

The truck, owned by Peninsula Sanitation and Recycling, was totaled and had to be towed from the scene.
